3-Hour Dengue Claim Decision*
Policies which includes life insurance coverage together with in-hospital benefit
To avail this special service:
Policy Owner or Beneficiary must send the below documents in soft/scanned copy, to 3hoursdengueclaim@metlife.com.bd by 12:30pm on a working day to be eligible for 3 hours claims decision process. Any claims request received after the cut-off time will be processed (decision making) on the next working day. Scanned or soft copies of the documents must be clear to read, any blurred document won’t be accepted.
Sickness claim by Individual Policyholder due to Dengue
- Accident or Sickness Claim Form for Individual Policy (Click here to download: English)
- Photocopy of Hospital Discharge Certificate with proper diagnosis, detailed case summary and daily follow-up notes along with issued itemized hospital bill (if any) that is signed by Hospital Authority.
- Photocopy of registered (minimum MBBS) Doctor’s prescription with proper diagnosis of sickness
- Dengue test report & photocopy of all diagnostic / investigation reports advised by the respective physician (Like X-Ray, MRI or CT Scan, etc.) (where applicable).
- Age Proof must be submitted. Any one of the following documents will be accepted as Proof of Age: (i. Photocopy of National ID Card/ii. Photocopy of Passport/iii. Photocopy of Driving License/iv. Photocopy of Certificates of S.S.C or equivalent examinations)
- Photocopy of a blank Cheque leaf (MICR) (For EFT payments only).
Death Benefit Claim by Individual Policyholder due to Dengue
- Duly filled Individual Claim Form
- Death Certificate and Dengue test report of Policyholder
- National ID card/ Passport/driving license/Birth Certificate of policyholder.
- National ID card/ Passport/driving license/Birth Certificate of Beneficiary
- Passport/Birth Certificate of Child for Juvenile/Child Policy
- Latest Policy Specification Schedule (PSS) and policy application form
- Scanned copy of a MICR cheque leaf or Bank Statement with Routing Code of Beneficiary
N.B. Company may ask for additional information and documents which will require more processing time.
Policy holder/ Beneficiaries are requested to send all documents via email only
